The information given will then be gathered and used to contribute to the Pharmaceutical Needs Assessment (PNA) to make decisions about where they are located, their opening hours, and the services they provide.
Please note that the end date for this survey is the 28th of March 2022.
There is also a separate survey that can be completed by pharmacies and providers of health and social care services here which will also close by the 28th of March 2022.
